Lifeguard Press charging kit recall

About 25,400 units of Lifeguard Press manufactured charging kits have been recalled due to six reported cases of the chargers emitting smoke and sparking. The U.S. Consumer Product Safety Commission found these devices to be unfit for use. The details are listed below:

Description

This recall involves seven models of charging kits with universal serial bus (USB) connectors that are used to recharge Apple iPhone, iPad and iPod devices. The recalled kits were sold under the Ban.do, Jonathan Adler and Lilly Pulitzer brand names and contained a wall charger, a 12 to 24-volt car charger, a 30-pin USB cord and a Lightning port adapter. The wall chargers are plastic cubes about 1 ¼ inches long, 1 inch wide and ½ inch tall with two metal prongs for plugging into an electrical outlet on one end and a USB port on the opposite end. Ban.do wall chargers came in the colors yellow and black and white stripe. Jonathan Adler wall chargers came in multicolor and blue and white patterns. Lilly Pulitzer wall chargers came in multicolor floral patterns. The following charging kits are being recalled:

Brand names are on the top front of the packaging and on the Lightning port adapters. Model names are on the bottom front of the packaging. Item numbers are on the back of the package in the bottom right corner.

Incidents/Injuries

Lifeguard Press has received six reports of the wall chargers emitting smoke and sparking and six reports of prongs detaching from the plug. No injuries have been reported.

Remedy

Consumers should immediately unplug and stop using the USB wall charger and contact Lifeguard Press to return the charging kit for a full refund.

Dillard’s, Lilly Pulitzer, Nordstrom’s and independent boutiques nationwide from February 2014 to June 2014 for between $25 and $30.

Ford Recall PTO pressure switches

Tactical Vehicle Outfitters is recalling Ford vehicles after detecting an issue with the PTO pressure switch. The 2010-2014 Ford F-350 and F-550 vehicles modified by Tactical and assembled with certain Muncie Power Products PTO Assemblies were found to leak oil onto a thermal blanket covering the exhaust. When combined with heat from the exhaust, the vehicle has an increased risk of fire causing injury and accident.

Tactical has agreed to install a new PTO pressure switch, free of charge beginning in July 2014. Other companies have made similar recalls: Stahl also recalled Ford’s 2009-2014 Ford F-350 Super Duty, F-450 Super Duty, and F-550 Super Duty vehicles with the same issues. Utility Truck Equipment Company has recalled their 2010-2013 UTLI41A, 2011 UTLN41, and 2012 UTLN46 aerial trucks manufactured December 2010 to September 2013. Somerset Welding & Steel, doing business as J&J Truck equipment (J&J), has recalled certain model year 2009-2013 Ford F-350, F-450, and F-550 Super Duty chassis with 6.8L gasoline engines modified by J&J to be equipped with certain Muncie Power Products PTO Assemblies.

Master Cutlery knife sheath recall

Master Cutlery Inc recalled about 4,000 knives and sheaths due to sheathing problems. The transcript is found below:

 

Description

This recall involves the Karambit neck knife and sheath. The half-moon shaped knife measures 7 inches in length and was sold in an urban camo print. “MTech USA” is laser printed on the blade. Model number MT-664UC-SO is marked on the back of the knife. The knife fits into an unmarked molded black sheath that is about 4.5 inches in length. The sheath has a snap clip on the back and also comes with a beaded lanyard chain. Replacement sheaths have “MTech USA” laser printed on the back.

Incidents/Injuries

None reported.

Remedy

Consumers should immediately stop using the recalled knife and sheath, store the knife and sheath in a safe area and contact Master Cutlery for instructions on obtaining a free replacement sheath.

KTM Enduro recall

The rear brake pedal mounting bolt of KTM North America’s 2014 690 Enduro R motorcycles was found to easily loosen and detach upon extended usage, increasing the risk of an accident. KTM has announced a recall of all Enduro motorcycles manufacture between October 2013 and April 2014. When the rear brakes dysfunctions, the rider must rely only on the front brakes to decelerate, causing the motorcycle to easily flip or skid.

The Charles E. Boyk Law Offices recently partnered with Josina Lott Residents and Lourdes University students in the creation of a Toledo skyline mural. The mural is in the PNC Building at the Boyk Law Offices.

The creation of the Toledo skyline mural is part of the Charles Boyk Law Office’s ‘Tribute to Toledo’ campaign. This campaign included residents of Josina Lott Residential Community Services and Lourdes University Art Therapy students. The groups came together to design the mural, as well as pain the skyline.

The beautiful ‘Tribute to Toledo’ mural is on the wall of the expanded area in the Charles Boyk Law Office. As supporters of the community that we serve, our office is very thankful to have been able to involve these groups in a project that not only brought people together and celebrated creativity, but also pays tribute to the Toledo area.

The Toledo mural painting was a day-long event for those who participated. As we watched the painting of the mural, it was amazing to see all of the pieces come together throughout the process. The participants from Josina Lott and Lourdes were then able to sign the mural as artists of the beautiful piece.

We believe that projects like this are very important to be a part of. It not only brings us closer to the community that we serve, but allows us to bring joy and unforgettable memories to others.

Teen Car Accidents: Deaths Spike during Summer

The National Safety Council has shared an infographic that highlights the summertime, between Memorial Day and Labor Day, as the 100 deadliest days during the year for teenagers. This is said to be the deadliest time of year due to teenager’s freedom during summer months to drive later into the night and to have more free time to hang out with one another, resulting in multiple teens in one vehicle.

Car accidents are the leading cause of death for teenagers in the United States. The three months with the deadliest days for teens include June, July and August. The NSC infographic states that in 2013, June had 327 deaths, July there were 319, and August there were 286 deaths. One fact that may be surprising to some is that more than half of the teenagers killed in car accidents are not wearing their seat belt at the time of the crash. Nursing Home Abuse: Overmedicating Patients

Overmedicating Nursing Home Patients

The JereBeasley Reports states that nursing homes are understaffed and those who are on staff are inadequately trained which can result in issues within the facilities. There is an issue that exists in which elderly patients are being given medications to control their behavior, including some that are very strong and can be dangerous. Instead of dedicating the appropriate resources to patients and residents, facilities are turning towards medications when they could instead spend the appropriate time with the residents to discover what the cause of certain behaviors may be.

The daughter of a patient in California recently learned first-hand what the nursing homes are doing when it comes to medications. Kathi Levine discovered that her 79-year-old mother received poor car throughout an 18 day stay at a nursing home. The patient went into the nursing home for phsycial therapy, and at the time could still hold conversations, dress and feed herself. When her daughter picked her up, she could not speak, was withdrawn and slumped in her wheelchair. The patient then died within weeks of leaving the nursing facility.

After this tragic end to a stay at a nursing home, the daughter, Kathi Levine learned that her mother was given a laundry list of medications while at the facility, including psychotics.

In May, lawyers working on Levine’s behalf, along with lawyers from the AARP Foundation agreed on a class-action settlement against the facility that administered Levine’s mother, stating “using dangerous drugs without the consent of residents or family members.”

Nursing Home Abuse

According to the JereBeasley Report, one in five patients is administered antipsychotic drugs that are not necessary and can be dangerous to the health of elderly patients. The drugs administered are even known to be dangerous to the elderly population, increasing the risk of death.

Why single-engine airplanes can be dangerous

Roughly 220,000 general-aviation aircraft are decades old. The average single-engine airplane in the Federal Aviation Administration registry is 41 years old. 50,000 of these airplanes were built more than a half-century ago.

Compared to automobiles in the U.S., this number is stark in contrast. The average automobile is only 11.4 years old and has to meet standards set from the date of manufacturing. Small aircraft, on the other hand, have an exemption policy called “grandfathering” that allows manufacturers to bypass any such safety regulation.

Greg Bowles of the General Aviation Manufacturers Association explains the severity of the issue: "If you were to own a car built 40 years ago, you wouldn't want to drive it every day. It wouldn't have anti-lock brakes, air bags or energy-absorbing crumple zones . . . That's the world of (general-aviation) airplanes as well."

Many amateur pilots continue using propeller-drive airplanes from decades ago due to the FAA advisory committee announcement that these airplane users would have to spend a significant amount more in order to meet current standards. With pilots flying old aircraft and not adding new protective features, "the safety level of general aviation does not improve," the 346-page report concluded. Surprisingly, “many amateur-flown aircraft lack rudimentary safety features such as shoulder belts, which the FAA and its predecessor delayed requiring in the 1960s, 1970s and 1980s, despite repeated studies showing they would prevent hundreds of deaths a year by keeping people from slamming into hard interior surfaces in a crash.”

The fact of the matter is, these airplanes are old and outdates and the FAA is doing little to change their policies. Sadly, crashes and accidents occur every year and have continued to remain constant.
